Life and AD&D Benefits
Basic Group Life and Accidental Death and Dismemberment (AD&D) coverage is a 100% employer paid benefit, provided at no cost to you. The basic life benefit will be paid to your beneficiaries in the event of your death. The AD&D benefit will be paid in the event of an injury resulting from a covered accident.

When your needs call for more than Basic Life and AD&D coverage, additional Voluntary Life and AD&D coverage is available for purchase for you, your spouse and your dependent children. Coverage purchased through this plan is offered at a group discounted rate with guaranteed issue amounts. Employees must elect coverage in order to purchase spouse or child coverage.
Annual Enrollment Period: As determined by Your Employer on a yearly basis.
During this period, the Evidence of lnsurability Requirement in the Policy is waived for benefit increases for late entrants listed below.
Allowable benefit increases are:
For currently enrolled:
• Employees: up to 1 increment of $10,000
• Spouses: up to 1 increment of $5,000
Benefit increases may not exceed the policy’s guaranteed issue amount.
This open enrollment does not apply if You or Your Spouse previously applied for coverage under the Prior Policy but were not approved or if You or Your Spouse were previously required to submit
Evidence of lnsurability but failed to do so. This open enrollment also does not apply to employees who have not satisfied the Eligibility Waiting Period.
This open enrollment also applies to child and Accidental Death and Dismemberment coverage. Evidence of insurability is not required for child or Accidental Death and Dismemberment coverage.
